school age enrollment 2011-2012

Follow the steps below to begin the 2011-2012 school age enrollment process. Please be patient when downloading forms as some may take a little time to download. Complete all required forms and submit, along with payment, to the YWCA front desk in order to complete your registration.

1. Print out the enrollment letter for the site you would like your child to attend:

    • Homer Elementary ENROLLMENT LETTER 2011-2012
    • Homer Intermediate ENROLLMENT LETTER 2011-2012
    • Parker ENROLLMENT LETTER 2011-2012
    • Randall ENROLLMENT LETTER 2011-2012
    • Smith ENROLLMENT LETTER 2011-2012

2. Print out the ENROLLMENT PACKET

3. Print out ALL three of the following forms:
    • Income Eligibility Application
    • Medical Statement of Child in Childcare
    • Written Medication Consent Form

4. Print Calendar & Parent Handbook for future reference:
    • School Age Parent Handbook 2011-2012
    • YWCA School Age Calendar 2011-2012

5. If you would like to apply to transport children from one Cortland site to another, please
    complete this form:
