school age enrollment 2007-2008

Follow the steps below to begin the 2007-2008 school age enrollment process. Please be patient when downloading forms as some forms take a little time to download.

ATTENTION: Correction to the school calendar. School begins WEDNESDAY, September 5th.

1. Print out the form for the site you would like your child to attend:
• Appleby ENROLLMENT PACKET 2007-2008

    • Hartnett ENROLLMENT PACKET 2007-2008
    • Homer Elementary ENROLLMENT PACKET 2007-2008
    • Homer Intermediate ENROLLMENT PACKET 2007-2008
    • Parker ENROLLMENT PACKET 2007-2008
    • Randall ENROLLMENT PACKET 2007-2008
    • Smith ENROLLMENT PACKET 2007-2008
    • YWCA ENROLLMENT PACKET 2007-2008

2. Print out ALL three of the following forms:
    • Income Eligibility Application
    • Medical Statement of Child in Childcare
    • Written Medication Consent Form

3. Print Calendar & Parent Handbook for future reference:
    • School Age Calandar 2007-2008 ATTENTION: Correction to the school calendar. School
    begins WEDNESDAY, September 5th.
    • School Age Parent Handbook 2007-2008

4. Complete all required forms and submit, along with payment, to the YWCA front desk in order to complete your registration.
