school age enrollment 2010-2011

Follow the steps below to begin the 2010-2011 school age enrollment process. Please be patient when downloading forms as some may take a little time to download. Complete all required forms and submit, along with payment, to the YWCA front desk in order to complete your registration.

1. Print out the form for the site you would like your child to attend:

2. Print out ALL three of the following forms:
    • Income Eligibility Application
    • Medical Statement of Child in Childcare
    • Written Medication Consent Form

3. Print Calendar & Parent Handbook for future reference:
    • School Age Parent Handbook 2010-2011
    • YWCA School Age Calendar 2010-2011

4. If you would like to apply to the YWCA (14 Clayton Avenue) site, please complete this form:
