summer camp enrollment 2009

Follow the steps below to begin the 2009 summer camp enrollment process. Please be patient when downloading forms as some forms take a little time to download.

1. Print out the appropriate Enrollment packet(s):
    • YWCA Summer Camp Enrollment 2009
    • YWCA Summer Camp CIT Enrollment 2009

2. Print out ALL four of the following forms:
    • Income Eligibility Application
    • Medical Statement of Child in Childcare
    • Written Medication Consent Form
    • CRMC Authorization for Emergency Treatment of Minors

3. Complete all required forms and submit, along with payment, to the YWCA front desk in order to complete your registration. The front desk staff cannot accept incomplete enrollment packets.
